Pdf Summary
The field of psychiatry is facing various challenges as it moves into the future, but it also presents opportunities for growth and innovation. The American Psychiatric Association's (APA) 2022 President's Workgroup on the 'Future of Psychiatry' has put forth recommendations for solutions to address these challenges. These recommendations include the future of the Diagnostic and Statistical Manual of Mental Disorders (DSM) and psychiatric diagnosis, clinical practice guidelines, the Psychiatric Patient Registry (PsychPRO), development of quality measures and standards of care, education, and policy development and advocacy.

PsychPRO, in particular, can make a difference by contributing to research and a better understanding of the brain and disorders. It can also support collaborative and integrated care models through registry partnerships. PsychPRO can improve access to care and address quality issues through data collection and analysis. It can also help support psychiatrists in their clinical practice and reduce stress and burnout.

However, there have been challenges with the development and implementation of PsychPRO, particularly with the previous vendor. The vendor focused primarily on the Medicare Access and CHIP Reauthorization Act (MIPS) instead of fully supporting grants. The patient/clinician portal built by the vendor was clunky and unsuccessful, and the fee structure was expensive for larger institutions.

To overcome these challenges, the APA is considering different approaches for the future of PsychPRO, such as outsourcing all functions to a third-party vendor, developing all functions in-house, or a combination of both. Each approach has its advantages and disadvantages in terms of implementation timelines, cost, expertise, and institutional memory.

In the short-term, the APA plans to focus on MIPS reporting and identify a new vendor for a sub-registry. They will also continue working with current participants to collect data and improve reports and dashboards. In the mid to long-term, they aim to build in-house technical capabilities and work with vendors to implement multiple data transfer methods. Eventually, depending on their in-house capability and registry participation, they may move towards full in-house functionality.

Overall, the APA's vision for the future of PsychPRO involves a stepped approach with metrics to evaluate the success of each step and make adjustments as needed. The focus will shift from MIPS to quality, and the ultimate goal is to improve the future of psychiatry through standardized data collection, analysis, and improved patient care.
